A COVID-19 vaccination consent form, which is a mandatory document for everyone who wants to receive a coronavirus shot, is available at kovid19vakcinacija.mk official webpage.
The consent form can be filled out before arrival at COVID-19 vaccination sites, though it’s also available at the vaccination sites, Health Minister Venko Filipche posted on Facebook.
He calls on all citizens to download and complete the consent form before the vaccination appointment, if possible, in order to avoid crowds and queuing at the vaccination sites.
